Abstract

Objectives: Literature review on the impact of stigma on mental health and how it affects treatment. Background and aims: In low- and middle-income countries, between 76% and 85% of people with mental disorders receive no treatment. In high-income countries, 35% to 50% of these subjects are in the same situation. One of the main reasons? The stigma. Stigma is a clinical risk factor for negative mental health outcomes, it causes delay in seeking and continuing treatment and can prolong the duration of untreated illness. Materials and Methods: Bibliographic review was carried out through the research in PubMed database using the following keywords: “mental disorder”, “family stigma”, “stigma impact”. Results: Stigma delays treatment seeking, worsens course and outcome, reduces compliance, and increases the risk of relapse causing further disability, discrimination, and isolation even in people who have access to mental health services. The delay in treatment can cause complications like suicide, violence, harm to others, and deterioration in capacity to look after one's physical health. Conclusion: Early intervention in mental disorders is not only neuroprotective but has the potential to arrest or delay deterioration and relapse. Stigma is the key barrier to successful treatment engagement, including seeking and sustaining participation in services. Unfortunately, the clinical impact of stigma remains a low priority in research. We reinforce the need to develop tools for assessing and include them in mental health assessment in order to investigate and approach to this risk factor.
