Background
Nigeria has the highest number of diabetic cases in Africa estimated to be 3.75 million with a prevalence of 4.6% as documented by the International Diabetes Federation. Complications of diabetes pose great management challenges with a resultant huge economic burden. Duration of illness and adherence to medications are key factors in the development of complications among diabetics.

Aim
The aim of this study was to assess drug adherence and complications of type II diabetes mellitus among patients in a secondary health institution in Ijebu-Ode, Nigeria.

Method
A cross-sectional study was carried out among all 200 adult type II diabetic patients currently on anti-diabetic treatment with age range between 30 to 79 years and gave informed consent to participate at the State hospital, Ijebu Ode, Nigeria; during the 6-week period allocated for the study. Data was collected using validated semi-structured, interviewer-administered questionnaire and examining case files of patients. Data entry was done using Microsoft Excel 2007 and analysed using SPSS version 17.0. Frequencies, percentages and inferential statistics (p<0.05 was considered statistically significant) were calculated and presented. Strict confidentiality was ensured and proper ethical protocols were followed.

Results
Duration of diagnosis of diabetes mellitus ranged from 1 to 380 months with a mean of 67.18±67.55 months. Their current drug therapy included Metformin 199(99.5%), Glibenclamide 37(18.5%), Glimepiride 85(42.5%), Gliclazide 25(12.5%) and Pioglitazone 100(50%). Glycaemic control was achieved in 51.5% of respondents. High adherence was found in 38.5%, medium adherence in 36% and low adherence in 25.5%. 11 complications were reported among the respondents with peripheral neuropathy being the commonest (64%). The adherence was significantly associated with duration of illness (p=0.01). Medication adherence rates reduced as duration of illness increased. However, adherence was not significantly associated with the presence of complications (p=0.66).

Discussion
Respondents’ adherence to medications was good but a high number of complications were reported among them. However, there was no significant relationship between adherence and complications. Management of diabetes should be multimodal and interdisciplinary to advocate drug adherence and optimum prevention of complications.
