Cerebrovascular disease is the second cause of death in the world, in Mexico, it represents the fourth cause. Based in the Guidelines of the WSO, AHA, and other organizations, we decided  to create an algorithm that make easier the reception and attention of patients who had a stroke. In Mexico there is a multiple systems of heath care. There are 4 principal types, one for people who has an employer and mensual salary, another one for those who does not have a formal job and regular income. The third one is for employees of the Federal Government (ISSSTE) and the last one is the Private Medicine. Our center, the Hospital Regional 1o de Octubre, is part of ISSSTE. It has been a very difficult enterprise to organize a system that brakes the paradigm that for many years had been establish around this disease. However, the new era of stroke has become a revolution in all the world, so we analyze the cost and effectiveness in our hospital, our city and country of the actual treatment (Thrombolysis alone versus the Endovascular Thrombectomy), and the result were presented to the administrative authorities of the ISSSTE. We had the approval to develop the endovascular treatment and budget to create the fist Stroke Center in the country. We treated 28 patients in 16 months, with an initial NIHSS average of 23, and an mRS outcome of 3 or less in 19 patients.
