Abstract

Background: Post-stroke depression is a common neuropsychiatric consequence of stroke that is challenging to identify in the inpatient setting. Nearly a third of all survivor’s experience major depression within the first three months after their stroke. Early evaluation and treatment of patients with post stroke depressive symptoms is key to the successful rehabilitation and recovery of stroke patients starting in the acute hospital setting and following through to the outpatient stroke clinic Method: In 2016, two multidisciplinary task forces were created and instituted a protocol utilizing speech- therapy lead stroke depression screening of all acute stroke patients admitted to Duke University medical center. Treatment guidelines were created based on the severity of depression scores on the PHQ-9. All discharged patients returning to the outpatient stroke clinic for follow up underwent repeat PHQ-9 screening at each of the follow up visits. Results: Between 9/1/2016 and 8/31/2018, 1,583 patients were admitted with acute stroke or intracerebral hemorrhage with 691 (44%) being able to participate in completing the PHQ-9 test. The average score among these patients was 15. During the same time line, 534 PHQ9 surveys were administered in the OP setting with an average score of 6. Conclusion: Screening for depression beginning in the acute care setting and following through to the outpatient stroke clinic is feasible and is able to identify patients at risk for depression. We observed a significant reduction in PHQ-9 scores between the acute setting and post rehab follow up.
