Recently, psoriasis is classified as an immune-mediated inflammatory disease (IMID) of the skin. In recent study, there is a result that the risk such as myocardial infarction significantly increases when the ratio of plasma eicosapentaenoic acid to arachidonic acid (EPA/AA ratio) falls below 0.4. On the other hand, it was known that fatty acid-binding protein (FABP) is one of the lipid chaperone families with a molecular weight of 14-15 kDa that binds to hydrophobic ligands such as a long chain fatty acid and eicosanoid. FABP have nine isoforms of which FABP4 is expressed in adipocytes and macrophages and is deeply involved in inflammation and intracellular lipid metabolism. In addition, FABP4 and FABP5 have a critical role in the maintenance, longevity and function of CD8+ tissue-resident memory T (TRM) cells, and suggest that CD8+ TRM cells use exogenous FFAs and their oxidative metabolism to persist in tissue and to mediate protective immunity. Therefore, we examined the EPA/AA ratio, CRP, serum IL-6, and TNF-α of 24 psoriasis patients (18 male and 6 female, 16 psoriasis vulgaris (PsO) and 8 psoriasis arthritis(PsA) ) at Dermatology clinic in Nagoya City University Hospital. We evaluated the correlation between PASI score and the ratio of EPA/AA. At first, in 16 PsO of all, PASI score is positively correlated with the ratio of EPA/AA, EPA and DHA (r=0.631, p=0.0087, r=0.84, p=0.00005 and r=0.623, p=0.45564), but also PsA (r=0.262, p=0.53092, r=0.262, p=0.53092 and r=0.310, p=0.00987). EPA and DHA are ω−3 fatty acids which have preventive effect to arterial sclerosis and myocardial infarction. Next, we found that CRP is positively correlated with AA which is called ω−6 fatty acids and IL-6 (r=0.531, p=0.03424, r=0.608, p=0.01238) in PsO. This results suggest that the EPA/AA ratio should be lower but by some chance, we should especially lower arachidonic acid value, because arachidonic acid might induce inflammation. A specific diet will influence the omega 3 and 6 fatty acids. From the long-term viewpoint, we should pay attention to lifestyle including diet toward psoriasis patients.
