Introduction: The implementation of best practices in cognitive rehabilitation post-stroke is challenging, as it involves complex and individualized interdisciplinary interventions. Further efforts are needed to develop effective knowledge translation interventions to support best practices. 
Study objectives: 1) To develop a tailored and multifaceted intervention to support the implementation of best practices in cognitive rehabilitation; and 2) To evaluate its impact on clinicians’ knowledge and intention to implement the best practices.   
Methods: Cases studies with interdisciplinary teams from two rehabilitation centers (n=11 and 14 clinicians). Each team prioritized one evidence-based practice. The intervention consisted of: provision of learning tools (i.e., videos, pocket cards and summary sheets for key stakeholders) created based on the results of rapid reviews and consultations with team members; and an interactive workshop to foster sharing of experiences and knowledge integration. Participants’ knowledge and intention to implement the practices were assessed using questionnaires at pre- and post-intervention and focus groups two weeks post-intervention.
Results: Participants’ knowledge about the key principles for applying these best practices significantly increased at post-intervention (p<0.05) Their intention to implement the practices remained high from pre to post-intervention (p≥0.05). Participants explained their intention to adopt the practices by the fact that it responds to an important need identified through team consensus.
Conclusions: This promising intervention addresses the need to support interprofessional teams in the implementation of best practices in cognitive rehabilitation post-stroke. The next step in this ongoing project is to evaluate the impact of this intervention on clinicians’ actual practices.
