Numerous diseases and pharmaceutical therapies have been shown to damage the small intestinal mucosa leading to changes in intestinal permeability. Examples include celiac disease, rheumatoid arthritis, HIV, food allergies and intolerance, and non-steroidal anti-inflammatory drug use. Measuring absorption of the metabolically inert carbohydrates D-mannitol, L-rhamnose, 3-O-methyl-D-glucose, D-xylose, and lactulose is a non-invasive method for evaluating intestinal permeability. In this work a high-performance anion-exchange with pulsed amperometric detection (HPAE-PAD) chromatographic assay was developed to directly quantify carbohydrates in urine. Linear ranges were evaluated for five carbohydrate probes as well as potentially present endogenous carbohydrates. Method precision, as relative standard deviation (RSD) of peak area, was <2% for each probe. Carbohydrate recoveries, measured by spiking prior to sample dilution, ranged from 91-115% across three days of sample analysis, indicating method accuracy. This assay allows carbohydrate probe determination without complex sample preparation, sample derivatization, or matrix elimination steps common to other methods.
