Background
Left atrial (LA) volume (LAV) is one of the key variables for evaluating left ventricular (LV) diastolic function. However, only LA anteroposterior diameter (LAAP) can be available in numerous large databases or epidemiology studies. Therefore, this study aimed to validate whether LV diastolic function could be evaluated with estimated LAV from LAAP.
Method(s)
A total of 346 inpatients were consecutively enrolled. LAV was measured by biplane Simpson’s method. LV diastolic function was evaluated according to 2016 recommendations. Best-fitting regression models of LAV-LAAP were developed and equations with the highest F-value were chosen in the first 173 subjects (learning set) and concordance for evaluating LV diastolic function between using estimated and observed LAV index (LAVI) was verified in the remaining 173 subjects (testing set).
Result(s)
In the learning set, linear model has the highest F-value in all subjects and in the subjects with reduced LV ejection fraction (LVEF), but compound, growth and exponential models in the subjects with normal LVEF. In the testing set, using the equation (Linear + Linear), the highest intraclass correlations coefficient and the highest κ coefficient (0.56) and overall proportion of agreement (79.2%) were shown betweem estimated LAVI and observed LAVI, and between estimated and observed LAVI > 34ml/m2, respectively. And concordance for evaluating LV diastolic function between using estimated and observed LAVI was good (κ = 0.80).
Conclusion(s)
LV diastolic function could be evaluated with estimated LAV from LAAP, which might provide a surrogate method when the direct measurement of LAV is not available.
