Purpose: The outcomes of out-of-hospital cardiac arrest (OHCA) at home are known to be worse that those at public locations. However, the association of detailed locations inside the house with the outcomes of at-home OHCA cases is unclear. This study aimed to study the association of detailed locations inside the house with outcomes of at-home OHCA cases. Methods: We analysed nationwide databases on 150,804 at-home OHCA cases between January 1, 2018 and December 31, 2019 in Japan using propensity score matching. Results: The one-month neurologically favourable survival rate of at-home OHCA was apparently the worst when OHCA occurred at that occurred at bath and dressing room locations: 0.52% (97/18,532) at bath and dressing rooms, 2.76% (268/9,700) at the stairs or hallways, 2.28% (2,373/103,885) at living/dining/bedrooms, and 1.87% (349/18,674) at other places (p <0.001). OHCA cases at bath and dressing rooms were very rarely witnessed (9.0% [1,63/18,532]) and had shockable rhythms (1.3% [237/18,532]). The adjusted odds ratio (95% confidence interval) of bath and dressing rooms for neurologically favourable survival, compared to other locations as a reference, was 0.66 (0.49–0.88) after a propensity score matching that included all epidemiologic factors and characteristics of at-home OHCA. In a multiple regression analysis only for OHCA cases at bath and dressing rooms, witnessed status (17.78: 10.85–29.13), initial shockable rhythms (7.50: 4.31–13.06), bystander cardiopulmonary resuscitation (2.00: 1.29–3.10), cardiac origin (0.60: 0.37–0.96) and patient age (0.96: 0.95–0.97) were major factors associated with neurologically favourable survival. Conclusion: Bath and dressing rooms were associated with lower rates of survival for at-home OHCA cases, even when epidemiology and characteristics were taken into consideration. Strategies to augment the chance of witnesses may be necessary for improving the survival of at-home OHCA.
