Purpose: Evaluate the changes in corneal endothelial cell density (ECD) after ab-interno gelatin microstent (Xen) implantation. There is currently no long-term data looking at ECD after Xen implantation. In light of ECD reduction 5 years post CyPass microstent implantation, such data would provide insights to Xen implantation.

Methods: Primary outcome was central corneal ECD after at least 1 year since Xen implantation. Secondary outcomes included interventions, complications, and reoperations.

Results: 55 ECD measurements in 52 eyes were done ranging from post-operative year 1 to 4+. Mean (SD) ECD at post-operative year 1/2/3/4+ were 1834 (510), 2076 (673), 1764 (568), and 1929 (588) with p = 0.388. There were 22 interventions (38% had needling), and 8 complications (most were transient). 13% received reoperation.

Conclusions: Post ab-interno gelatin microstent implantation did not show a significant reduction in corneal endothelial cell density at post-operative year 4 when compared to post-operative year 1. Additional ECC measurements will be collected prior to finalization of abstract.
