Purpose: To compare the Panoptix™ and Symfony® multifocal intraocular lenses (IOLs) with respect to patient  satisfaction,  spectacle dependence, visual acuity (VA), dysphotopsias, and best corrected mesopic contrast sensitivity  
Study Design: Non-randomized, retrospective, single -centre, comparative study.
Methods: Twenty four subjects (mean age 68 years) implanted in both eyes with either the AcrySof IQ Panoptix™ IOL (n=11) or TECNIS Symfony®IOL (n=13) between January 2015 and April 2019 were recruited and  assessed 3 months to 4 years after their second surgery. Choice of IOL  was based on surgeon’s discretion and patient preference.  The following outcomes were evaluated: patient satisfaction, spectacle dependence, dysphotopsia,  binocular and monocular best corrected VA at distance , binocular and monocular uncorrected  VA at distance, 40cm and 60cm, and monocular  best corrected mesopic contrast sensitivity. 
Results: The mean age was 68 years. There were no statistically significant differences in  baseline characteristics between the two groups. There was no difference  between the two IOLs for near or distance vision satisfaction: both groups had a median score of 10/10 for  overall distance vision,  and  7/10 and 8/10 for near vision  for Symfony® and the PanOptix™ , respectively. Binocular uncorrected  VA at 40cm in the PanOptix™ group was significantly better than the Symfony® group (p = 0.007). Mesopic best corrected contrast sensitivity with the Symfony® IOL was significantly better than with the PanOptix™ IOL at the higher spatial frequencies (p = 0.001).  There was no significant difference in dysphotopsia symptoms.
Conclusions: For both groups, satisfaction was excellent for distance vision and good  for near.   Binocular uncorrected near VA  was significantly better with the PanOptix™ IOL. Best corrected contrast sensitivity under mesopic conditions was significantly better with the Symfony® IOL at the higher spatial frequencies. The PanOptix™ IOL may be a better choice for those who want to optimize  uncorrected near VA, and the Symfony® IOL may be a better choice for those who want to optimize their mesopic contrast sensitivity .
