Soft and hard tissue modifications around tissue-level Laser-Lok implants

Laser-Lok is a promising computer-controlled laser ablation technique which creates an implant neck with precision-engineered cell-sized microchannels (size 8-12 micron). This neck micromorphology may have the potential to achieve/create a connective tissue attachment that may prevent apical migration during initial healing phases. The placement of these implants by using a flapless technique may lead to stable Marginal Bone Level (MBL). This preliminary study aims to evaluate soft and hard tissues modification around implants with Laser-Lok microtextured neck placed at tissue level with flapless technique. MBL and gingival parameters will be evaluated up to 6 months. Healthy, non smoker patients were enrolled in this prospective clinical study. Tapered tissue level implants (Biohorizon, Birmingham, AL, USA), characterized by 1.8mm Laser-Lok neck were used. The implant shoulders were placed levelled with the soft tissues to obtain a tissue level insertion and using a flapless technique. Customized abutments and provisional crowns were applied after 3 months. Definitive metal-ceramic crowns were cemented after 4 months. Periapical Rx were taken at implant insertion, 1,3 months (pre-loading) and 6 months (post-loading). MBL was evaluated in single blind using Image J software (NIH, Bethesda,USA). Plaque score (PS) and Bleeding on Probing (BoP) will be monitored at 4 sites around the implant restoration (Mesial, Distal, Lingual, Vestibular) at 3 months (before prosthetic phases) and at 6 months. Dichotomous scores were used (0=absence, 1=presence) for both parameters. Gingival biotype (thin/thick) was also recorded. A total of 15 implants were placed in 13 patients (9F;4M, mean age 59±8 years). After 6 months, all implants were safe from complications. A thick gingival biotype was found in 8 implant sites, while thin biotype was found in 7. After 6 months, extremely low BoP was observed (4.16%) while PS was positive in 9.5% of the cases. Implant distribution and MBL according to operative parameters (gender, location, implant placement timing, diameter and gingival thickness) is reported in Table 1. Mean MBL (expressed as mm ± SD) was -0.07 ± 0.14 at 1 month, -0.15 ± 0.24 at 3 months and -0.26 ± 0.17 at 6 months. Our preliminary data showed that tapered tissue level implants may be placed at tissue level by using a flapless approach. A stable crestal bone with minimal bone loss was observed during healing phases. Healthy soft tissues with low signs of inflammation were observed around Laser-Lok microtextured neck as confirmed by low BoP and PS.
