Transfusion of fresh frozen plasma is still a widespread therapy for dilutional coagulopathy although evidence for its efficacy is low. Prospective clinical studies suggest that plasma free strategies based on coagulation factor concentrate (CFC) administration guided by thrombelastometry parameters might be a superior alternative. We designed a colloid fluid based on human albumin enriched with CFCs that shows the same coagulatory characteristics in thrombelastometry as plasma. When combined with platelets Rotem parameters were comparable to whole blood.
In massive bleeding scenarios these kind of normocoagulant colloid solutions might be a feasible substitute for plasma.
