Purpose: Survival after one year is considered as a challenge by ILCOR 2015 Consensus Statement. However, we decided to undertake this challenge to better comprehend the long-term issues of survivors of out-of-hospital-cardiac arrest (OHCA). 
Methods: All the patients who suffered an OHCA in our Province (about 550000 inhabitants) are enrolled in the Cardiac Arrest Registry of the Province of Pavia. We evaluated survival at 1 month, and then every year until 5 years after OHCA.
Results: In the first three year (2015-2017) 1403 confirmed OHCA (male 60.3%; mean age 73.4±15.8 years) in which CPR was attempted have been collected. The mean EMS response time was 11:31±5:10 mins. 79.3% of OHCA occurred at home, 9.8% in public location, 8.6% in nursing facilities and 2.3% in other locations. 57.1% were bystander witnessed and 16.2% EMS-witnessed, whilst the other were no-witnessed. Bystander CPR was performed in 39.9% and an AED used before EMS in 2.9%. The aetiology was medical in the vast majority (93.1%), followed by traumatic (4.4%). The first rhythm was shockable in 18.4%, non-shockable in 81% and unknown in 0.6%. The survival according 2015 Utstein recommendation is presented in Table 1. 
Conclusions: Survival after an OHCA change over time, so we demonstrated that a long-term follow-up is useful to better comprehend long-term issues of cardiac arrest patients and to improve their treatment.
