Background
Large infarct core volume (CV) has been associated with poor outcome, however there is no evidence if the outcome would be different depending on the affected hemisphere. We hypothesized that the cutt-off CV to predict favorable outcomes could be different depending on the hemisphere stroke side.
Objectives
The aim of this study was to analyze if the outcome varies in  relation with the CV diffusion-weighted (DWI) in ischemic strokes according on hemisphere location.  
Materials and methods
In this retrospective study 108 consecutive patients with anterior circulation acute ischemic stroke with large vessel occlusion and basal magnetic resonance imaging who received endovascular treatment within 24 hours were included. Patients were stratified by stroke side in two groups left (L) and right (R). DWI ischemic CV were manually measured by two neuroradiologists using Osirix MD 7.5. Areas under the curve (AUCs) were used to evaluate the difference according to the volume of the infarct and the lateralization of the stroke to predict good outcomes (modified Rankin Scale mRS 0-2) at 90 days.
Results
Of the 108 patients, 60 were left and 48 right-sided strokes. The AUCs for DWI core volume in both hemispheres for probability of good outcome was 0,701 ± 0,051 cut-off core volume of 43 ml (odds-ratio 6,3 95% CI 2,6-15,4  p 0,00002) for right sided-group was 0,691 ±0,084 cut-off 63 ml (odds-ratio 9,2 95% CI 2,1-39,9 p 0,001) and 0,727 ± 0,065 cut-off 24 ml for left group (odds-ratio 6,8 95% CI 2,0-22,4 p 0,001). 
Conclusion
Cut-off value of CV for good outcome was different for each stroke side-group in our series. Patients with right-sided strokes tolerated larger core volume DWI infarcts with better outcomes.
