SAFETY AND EFFICACY OF ENDOVASCULAR TREATMENT OF ACUTE ISCHEMIC STROKE IN YOUNG PATIENTS

AIM AND OBJECTIVES:  To evaluate safety and efficacy of intra arterial thrombolysis (IAT), mechanical thrombectomy (MT) and thromboaspiration for treatment of acute ischemic stroke (AIS) among patients less than 18 years of age, presenting within 16 hrs of stroke onset.

MATERIALS AND METHODS: Patients treated for acute ischemic stroke by intra arterial thrombolysis, Mechanical Thrombectomy and thromboaspiration between January 2015 to June 2018 in our centre were evaluated retrospectively. Among them, Six patients were below 18 years of age. The patients were clinically and neurologically evaluated. Subsequently subjected to stroke protocol neuroimaging with CT, MRI and four vessel digital subtraction angiography (DSA).

RESULTS: Patients presenting within 16 hours of stroke onset, were given option of IAT, MT or Thromboaspiration and explained about the possible outcome and complications of the procedures. Angiographic evaluation demonstrated Middle Cerebral artery (M1) occlusion in 4 and ICA occlusion in two of them. Four patients treated by MT, one patient by thrombo-aspiration and other patient by IAT. 

Thrombolysis in Cerebral Infarction (TICI) 3 flow could be established in 5 patients. The sixth patient had recurrent occlusion despite repeated recanalization. This patient was treated by MT. mRS score at 90 days were zero for 4 patients, two for 1 patient, one for 1 patient. Patient with repeated re-occlusion had mRS score 2.

  CONCLUSIONS:  Mechanical thrombectomy, Thromboaspiration and IAT  are safe and effective procedures in acute ischemic stroke in patients less than 18 years.
