Objective: Current observational studies indicate that helicobacter pylori (HP) infection is associated with increased risk for stroke onset. Whether HP infection is associated with prognosis is unclear. Our purpose was to examine HP infected with higher IgG levels were increased the risk for poor prognosis outcome after 1 year onset.
Methods: From Jun. 2011 to Dec. 2013, 1150 ischemic stroke patients were recruited from 14 hospitals at 7 provinces in China and their baseline serum HP-IgG levels were tested. There were prospectively followed up for death, major disability and vascular events for 1 year after acute ischemic stroke. Cases were divided into 4 groups with quartiles of HP-IgG levels （quartile 1 to quartile 4）. A multivariable Cox regression models were used to estimate the association between HP-IgG levels and clinical outcome in cases.
Results: at the 1 year follow-up, the incidence of all-cause death, cardiovascular events, cardiovascular death and cardiovascular events or death were 4.83%, 7.90%, 3.72% and 11.43% respectively. After adjustment for multiple factors compared to lowest level of HP-IgG with Cox regression, highest level of HP-IgG were significant increased risk for all-cause mortality and cardiovascular death (HR 3.43, 95%CI 1.26-9.36; HR 5.45, 95% CI 1.57-18.90). The risk of cardiovascular events or death were increased in quartile 2, quartile 3, and quartile 4 (HR 1.84, 95%CI 1.02-3.34; HR 2.00, 95% CI 1.12-3.56; HR 1.97, 95% CI 1.10-3.55) respectively. The risk was elevated with the level of HP-IgG increased (P<0.05).
Conclusions: HP infection is the risk for ischemic stroke prognosis. Higher level of HP-IgG might increase the risk of mortality, cardiovascular death, cardiovascular event or death to ischemic stroke at 1 year onset.
