OBJECTIVE: Establish right-left shunts in patients with migraine with aura and stroke.   
BACKGROUND: The presence of shunts in migraine is a finding that can be established through non-invasive procedures, including transcranial Doppler with microbubbles (DTC MB). The study of microbubbles is highly sensitive and inexpensive. Identifying shunts can be associated as a predictor of stroke risk in a young population.
PATIENTS AND METHODS: Longitudinal descriptive study of 28 migraine patients with aura, migraine with persistent aura and migraine with aura and stroke from 2010 to 2018 using modified protocol of Venice, using EMS 9U2 equipment, with bilateral monitoring.
RESULTS: We reviewed 28 patients between 15 and 50 years old, 20 women and 8 men: 16 patients presented migraine with aura, 5 patients persisted with migraine aura without stroke and 7 patients with ischemic stroke and migraine. We identified Shunts in: migraine with aura 2 cases (22.2%), migraine with persistent aura 4 cases (44.4%) and ischemic stroke with migraine 3 cases (33.3%). We established categories: 2 cases (25%) corresponded to category 2, 2 (25%) category 3 and 5 (50%) category 4, we measured mean velocity and pulsatility indexes of right and left middle cerebral arteries obtaining MV of LACM: 40- 87cm/s, RACM: 41-69cm/s, PI LACM: 0.69-1.18 and IP RACM: 0.13-1.09.
CONCLUSIONS: Ultrasound findings showed the presence of shunts more frequently in patients who presented ischemic stroke as well as in the population of patients who had persistent migraine auras without stroke, this is a group of patients with higher risk of stroke. The utility of the MB DTC to identify the shunts in the patient group was adequate and easy; it is a method that is necessary to continue implementing in developing countries to address a patient population that may develop this complication. It is necessary to carry out studies with larger populations of patients and control cases to improve these results.
