Objectives: To report a possible case of first psychotic episode as the inaugural manifestation of a catecholamine-secreting tumor, while summarizing previous case reports regarding this matter. Background:Catecholamine-secreting tumors are rare tumors that arise from chromaffin cells of the adrenal medullaand the sympathetic ganglia. Although there are multiple cases highlighting the co-occurrence of catecholamine-secreting tumors and psychosis, the relation between the two remains unclear.Materials and methods: We conducted a non-systematic review on psychosis in pheochromocytoma and paraganglioma in humans and reported a possible case of catecholamine-secreting tumor and psychosis. Results:A 34-year-old male, with no previous history of psychiatric symptoms, was admitted in our ward after a sudden onset of persecutory and mystic delusions. During his stay, paroxysms of hypertension, tachycardia, palpitations, headache, hyperhidrosis, anxiety and nausea were observed. The patient exhibited elevated serum and urine metanephrines and abdominopelvic CT scan revealed two possible bilateral ganglia near the external iliac chain. The patient was referred to Endocrinology follow-up after discharge and is currently awaiting a definitive diagnosis. We also summarize previously reported cases. Conclusions:There are several cases reporting the co-occurrence of catecholamine-secreting tumors and psychosis. Furthermore, in some cases, the surgical resection of these tumors was followed by remission of both the psychiatric symptoms and hypertension. Although no clear relation between catecholamine-secreting tumors and psychosis has been established, further research in this area is needed. Moreover, clinicians should be aware of these rare tumors in psychiatric patients, since known pheochromocytoma is an absolute contraindication for electroconvulsive therapy.
