Background and Aims:
Atherosclerotic begins early in life and prevention strategies should not be delayed. However, cardiovascular (CV) risk estimation is underestimated in young population finding many patients with low short-term CVrisk but high CVrisk along life time (LTR). The aim is the identification of a urinary profile with added value in CVrisk estimation of young population and investigate its modulation with age.
Methods:
Urine samples were collected from 225 subjects aged 30-50 years, 50-70 years and >70 years, classified according to their CV risk in 3 groups per age range: “control”; CVrisk “factor”; and previous CV “event”. Proteomic and metabolomic differential analysis was performed by isobaric labeling-mass spectrometry and nuclear magnetic resonance. Selected candidates were confirmed by target analysis (MS) and ELISA.
Results:
The proteins ADX, ECP, FETUB, GDF15, GUAD and NOTCH1 compose a fingerprint linked to CVrisk, positively correlating with LTR. Multivariate ROC curve obtained for ECP, GDF15 and GUAD resulted in AUC=0.96 to separate control and CVrisk factor groups. Only FETUB showed the same variation in the middle-age group (50-70). The metabolomic analysis resulted in a sub-group of 32 metabolites showing alteration linked to CVrisk and being their response modulated along life. In particular, 12 metabolites uniquely correlated with CVrisk in young people, four metabolites in middle age, and two metabolites showed the same alteration pattern in the three ageranges
investigated.
Conclusions:
A novel urinary signature modulated by age is here shown as a potential tool to improve CVrisk estimations, treatment thresholds for intervention and prevention strategies design.

04 - MOLECULAR PROFILE OF CARDIOVASCULAR RISK IN YOUNG POPULATION: A PROMISING TOOL FOR EARLY PREVENTION
