Background: 
The incidence of diabetes and its associated morbidity and mortality is increasing world-wide. In Singapore,from 2008 to 2013, the rate of diabetes-related major lower extremities amputation increased from 11.0 to 13.3 per 100,000 population, this translates to approximately 4 amputations per day. In parallel, the rate of 30-day mortality after major lower extremity amputation increased from 6.0 to 11.1 percent with a 1-year mortality increasing from 24.3 to 30.6 percent. 

There is an urgent need to prevent and retard progression or development of a foot wound by addressing all aspects of the individual’s care needs. A pilot clinical service, a one-stop podiatry and nurse-led foot clinic, was initiated in a tertiary centre of the National University Hospital (NUH) Singapore.
