Background 
Exercise plays a crucial role in the management of T2DM. Despite tremendous advances in the hospital-based exercise intervention, exercise management for discharged patients remains challenging. In order to avoid failure or interruption of exercise intervention outside hospital, an integrated hospital-gym based, physician-directed and coach-supported exercise management model is put forward innovatively. Moreover, although artificial intelligence and Internet have been widely applied in diabetes care management, it is controversial whether manual intervention is negligible in exercise management.
Aims
The aim of study was to verify the preliminary efficacy of this integrated hospital-gym based exercise intervention model, with comparing the integrated manual-intelligent approach (experiment group) and the purely intelligent approach (control group).
Method
Seventy-nine discharged patients with T2DM were assigned to the experiment group (n=48) or to the control group (n=31). Both groups received the same exercise protocol composed of a tri-weekly low-intensity resistance exercise and a moderate-intensity aerobic exercise on the following day. Preliminary prescriptions were both created by a specific system based on an intelligence algorithm, but prescriptions for the experiment group were adjusted by qualified clinicians and exercise specialists. Moreover, the experiment group was instructed and supervised mainly by face-to-face coaches in gyms, while the intervention for the control group was delivered via intelligent App through Internet. The primary outcomes were weight, glycemic control and lipid profiles after 3-month intervention.
Results
After 3-month intervention, significant improvements in weight, HbA1c, FBG and TC (P<0.05) were found for both groups, but the integrated manual-intelligent approach show higher efficacy(Table 1& Figure 1)
Discussion
This study is an exploration of an multi-component, intelligent technology-supported diabetic exercise intervention model. The findings will provide evidence to develop an efficient and long-term management of diabetes exercise.
