Purpose: The purpose of our study was to investigate the safety profile and efficacy of micropulse transscleral cyclophotocoagulation (MPCPC).

Methods: Retrospective review of 49 patients undergoing MPCPC with one surgeon (PG) between July 2016 and August 2017. We recorded intraocular pressure, number of medications, daily diamox use, visual acuity (LogMar), and visual field parameters. This information was reassessed at post-operative week 1, post-operative month 1, 3, 6, 12, 18, and 24. Visual acuity data was collected pre-operatively, one year, and two years. Anesthesia was delivered with sub-conjunctival block. Laser delivery was for 80 seconds with a duty cycle of 31.3% at 2000 mW. Delivery site was inferior or superior conjunctiva or for 360 degrees.

Results:   Preoperatively patients were on a mean of 2.6 glaucoma medications. Twenty-four patients were on an average daily dose of diamox equal to 420 mg. Average intraocular pressure (IOP) was 24 mmHg. At two years follow up the average IOP was 17 mmHg (29% reduction). Eight patients remained on diamox (average daily dose 313 mg). However, in order to achieve IOP control, 26 (53%) of patients required additional micropulse CPC treatments over the 2-year period and 14 (29%) required a glaucoma surgery, either angle based or bleb forming. 

Conclusion: Our results indicate that the clinical efficacy of MPCPC may be lower than that reported in the literature. MPCPC is reasonably safe with low complication rate and can be considered in patients with vision better than 20/200 and should not be reserved for low vision patients.
