We present a case of recurrent pediatric retinal detachment in which the retina was successfully reattached to illustrate surgical techniques that may be associated with anatomic and functional success. A 12-year-old boy underwent cataract surgery complicated by a macula-on retinal detachment in the left eye. After his first reattachment surgery, he developed posterior capsule opacification and intraocular lens dislocation for which he received a YAG capsulotomy. Four weeks later, he presented with a total retinal detachment, proliferative vitreoretinopathy, and pre-retinal membranes. To increase the likelihood of anatomic success in the second repair, we performed a combined vitrectomy and scleral buckle, peeled pre-retinal membranes to release traction, used intraoperative perfluoro-n-octane to facilitate retinal reattachment, created chorioretinal irritation with a 360 degree barrier laser, and infused Densiron oil for post-operative tamponade to provide persistent attachment at follow-up. Four months after surgery, his vision was 20/80 and the retina remained attached. This case illustrates surgical techniques that may be associated with anatomic success as persistent retinal attachment may lead to improved long-term visual outcomes.
