Chordoma is an uncommon malignant neoplasm arising from notochord remnants and carries a poor prognosis with a high local recurrence rate. Although chordoma carries many characteristic imaging features it is primarily known as a slow growing but locally aggressive neoplasm. Metastatic disease is rare but when it does develop occurs most frequently to the lungs, followed by soft tissue/bone, and liver. Hematogenous spread of disease is exceeding rare and to our knowledge this is the only reported case of Chordoma tumor thrombus after an extensive literature search. This excerpta describes a case of metastatic Chordoma presenting with SVC syndrome and how the knowledge changes patient management.
