Developing rapid screening tests for diseases in complicated matrices such as whole blood without pretreatment steps will benefit populations that live beyond the reach of urban hospitals and medical infrastructure. Here we describe an instrument-free, rapid, and inexpensive single-step sink/float test that allows for biomarker detection from biological fluids using only the bare eye. This novel approach uses a polymeric biofunctionalized sphere as a substrate (solid support) for capturing the analyte biomarker, and superparamagnetic nanobeads as detection labels. The entire test including sample incubation and analyte detection can be performed in a short time of approximately 20-40 minutes. This sink/float immunodiagnostic test was successfully demonstrated for the one-step detection of Syphilis in whole blood, and for a semi-quantitative assessment of C-Reactive Protein in the range of 1-30 ng/mL in serum. We believe that the sink/float approach can be used as a means to diagnose diseases and clinical biomarkers of major significance in resource-limited and point-of-care settings.<br/>
