Introduction Type 2 Diabetes (T2DM) is associated with increased levels of cardiovascular disease and death, and high glucose levels after eating have been related to increased cardiovascular risk.Methods A pilot double-blinded crossover randomized controlled trials was undertaken with high and low polyphenol chocolate given to subjects with T2DM 60 minutes prior to a glucose load to simulate glucose levels following a meal. Efficacy was assessed by change in functional (endopat 2000) and biochemical endothelial function markers, and oxidative stress (urinary isoprostane) parameters.Results There was an improvement in functional endothelial cell response (reactive hyperemia), and endothelial serum markers iCAM, e-selectin, PSGL-1, and an improvement in oxidative stress following high polyphenol chocolate ingestion (Table 1) Conclusion This study suggests that high polyphenol chocolate ingestion prevents acute endothelial dysfunction and oxidative stress in T2DM following elevation of glucose modeling a simulated meal. Perhaps it is best to have pudding first!
