The photo-Fenton process has been long proposed as a potential disinfection technique for water and wastewater, especially for developing countries or sunny regions of the world. In this work, based on our experimental findings over the last 10 years and a critical review of the relevant literature, we present the different pathways involved in bacterial and viral disinfection during the application of the photo-Fenton process in wastewater at neutral pH. The differences between two model microorganisms’ inactivation patterns (<i>E. coli</i> and MS2 coliphage), due to the participation of the endogenous and exogenous pathways, are assessed. Furthermore, attributing the effects of the homogeneous and heterogeneous fraction of the photo-Fenton process is provided, alongside with identification of the implications due to the presence of organic matter, either of synergistic or antagonistic nature. Finally, the key targets for each microorganism are explored (genomic, proteomic, transcriptomic, etc.) and how inactivation is achieved via these routes.
