Background
Obstructive sleep apnea (OSA) is a sleep-related breathing disorder that has been shown to be an independent risk factor for cardiovascular mortality and morbidity. Postulated pathophysiologic consequences of OSA such as increased sympathetic activity and hypoxia leads to increased pulmonary arterial pressure, hypertension and heart failure. With the recent advancements and availability of echocardiography, relationship of right and left ventricular systolic and diastolic function is now increasingly investigated on its relationship with severity of obstructive sleep apnea. There is paucity of local data on the correlation of OSA severity and echocardiographic data such as left and right ventricular systolic function, and left ventricular diastolic function. This research will help the clinician have a better understanding of the prognosis of OSA.
Method(s)
This is a cross-sectional study from January 2016 to January 2017. Recent transthoracic echocardiography of Filipino patients, newly-diagnosed with obstructive sleep apnea (OSA) determined by apnea-hypopnea index (AHI) after an overnight polysomnography were reviewed. Echocardiographic indices such as left ventricular systolic function (global longitudinal strain and ejection fraction), right ventricular systolic function (tricuspid annular plane systolic excursion (TAPSE) and fractional area of change (FAC), left ventricular diastolic function and pulmonary pressures were evaluated using standard echocardiography.
Result(s)
A total of one hundred and fourteen, Filipino, newly-diagnosed obstructive sleep apnea (OSA) patients who had their echocardiography within six months from diagnosis of OSA and had no intervention (such as Continuous Positive Airway Pressure [CPAP]) were included. The patients were predominantly male (81%), obese, and belonged to the severe OSA category. Age and co-morbidities were similar across groups. Compared to the mild and moderate OSA, severe OSA had significantly more patients who were obese (p < 0.001). The severe OSA group had lower ejection fraction and GLS values and higher severity in diastolic function. There was insufficient evidence to show a difference in RV systolic function across OSA groups. Conclusion(s)
There is preponderance of male and obese patients in the severe OSA group. Severity of OSA is significantly associated with reduced left ventricular systolic function and higher severity in left diastolic dysfunction. Other parameters such as RV systolic function and pulmonary pressures are insignificant.
