Background: This study aimed to determine the risk of hypertension (HTN) on incidence of out-of-hospital cardiac arrest (OHCA) and to investigate whether the effect of HTN depends on anti-hypertensive medication.
Methods: This study was case-control study using the Cardiac Arrest Pursuit Trial with Unique Registration and Epidemiologic Surveillance (CAPTURES) project and Korea National Health and Nutrition Examination Survey (KNHANES) database. Cases were defined as EMS-treated adult OHCA patients with presumed cardiac etiology collected at 27 emergency departments from January to December 2014. Four controls were matched to one case with strata including age, gender and county from the KNHANES database. Multivariable conditional logistic regression analysis was conducted to estimate the effect of HTN and anti-hypertensive medication on incidence of OHCA.
Results: Total 1,466 OHCA patients and 5,864 community-based controls were analyzed. A total of 662 (45.2%) among OHCA cases and 3,190 (54.4%) among control were diagnosed with HTN. HTN diagnosis was associated with decreased risk for OHCA (0.69 (0.60-0.80)). In terms of anti-hypertensive medication, AOR (95% CI) was 0.64 (0.55-0.75) in medication group and 1.12 (0.83-1.49) in non-medication group.
Conclusion: Anti-hypertensive medication for HTN patients may help reduce the incidence of OHCA. The importance of active diagnosis of HTN and medications to reduce the incidence of cardiac arrest should be further emphasized.
