Aim: To investigate the association between sex and the overall survival to hospital discharge (S2D) of those patients who sustain Out-of-Hospital Cardiac Arrest (OHCA) in England.
Methods: A retrospective cohort study including OHCA patients of all ages for which resuscitation was attempted by participating NHS ambulance services in England between 1st January 2013 and 31st December 2018. Logistic regression analyses were performed to determine the effect of sex in OHCA outcome.
Results: 164,279 OHCA patients were included in the OHCAO project within the study period. Male accounted for 63.74% of all OHCA patients included. Female OHCA patients had more unfavourable Utstein characteristics: older (median age 75.6 years vs. 70.1 years, p<0.001); less likely to have OHCAs in public places (11.1% vs. 23.1%, p<0.001) and less likely to present in a shockable initial rhythm (14.9% vs. 27.4%, p<0.001). Although there is an unadjusted disadvantage for females in the overall survival to hospital discharge (crude OR= 0.60, 95% CI: 0.58-0.63, p<0.001), there is no difference in the survival to hospital discharge rates between men and women (adjusted OR=1.02, 95% CI: 0.91-1.16, p=0.687) after adjusting for age, level of deprivation, cardiac arrest location, witnessed status, bystander CPR status and initial cardiac arrest rhythm.
Conclusion: There is no difference in the overall survival between men and women, after adjustment for less favourable Utstein characteristics in females. Further research is required to explore the observed differences in the incidence of OHCA; the in-hospital survival between males and females and to understand how other unreported factors may influence the survival following OHCA in England.
