Purpose.
The association between the magnitude of stress hyperglycaemia (SHG) or glycaemic variability (GV) and neurological and vital outcomes has been reported, but tight glycaemic control does not seem to improve the prognosis. The relation between SHG or GV and neurological and vital outcomes in OHCA patients is uncertain too. The aim of this study was to assess the independent effect of SHG and GV on the long-term outcomes on a large cohort of OHCA patients.

Materials and methods.
The blood glucose (BG) values recorded during the 24 first hours were analysed in patients treated by the MICU (mobile intensive care unit) and admitted in the Saint-Pierre or Erasme hospitals after OHCA with ROSC between 2015 and 2020. SHG and GV were respectively assessed by the maximal and mean blood glucose (MBG and mBG) and the absolute glycaemic variation and mean absolute glucose change (MAGV and MAGC). Their association with 30-day Cerebral Performance Category (CPC) Score and mortality was evaluated through univariable and multivariable regressions.

Results.
In total, 146 patients were included (males 74%, age 61 + 15 years). The indices of SHG and GV were associated with worsened outcomes by univariable but not by multivariable analysis (table).

Conclusions.
These findings suggest that post-OHCA SHG and GV are related to the magnitude of stress and may not require a specific treatment.
