【Purpose】
The optimal resuscitation time in the field of out-of-hospital cardiac arrest (OHCA) has been debated. The purpose of this study was to evaluate the association between the time intervals from emergency medical services (EMSs) contacting patients to departure the scene (‘time on scene’) and the 1-month survival after OHCA.
【Methods】
This cohort study extracted adults <15y.o. with OHCA Using All-Japan Utstein data from 2018 to 2019. We included 172,786 patients treated by EMS staff. Patients were divided into shockable (n=10,222) and non-shockable (n=162,564) cohorts by initial electrocardiogram rhythm. The time-dependent propensity score was used to match cases that departed the scene within the same time as the casualty departed the scene in the time elapsed since casualty contact (every minute). We calculated the propensity scores as the time-varying probability of departure to the hospital using the multivariable logistic regression. One-to-one nearest neighbor matching was performed using the time-dependent propensity score. After matching, multivariate logistic regression analysis was performed to estimate the adjusted odds ratio (AOR) and 95% confidence intervals (CIs) for 1-month survival.
【Results】
In the Shockable cohort, 7 minutes of field activity was significantly positively associated with 1-month survival (AOR, 1.30; 95% CI, 1.01-1.67), and 17 minutes was significantly negatively associated with 1-month survival (AOR, 0.52; 95% CI, 0.27-0.96). In the non-shockable cohort, 11 minutes of field activity was significantly positively associated with 1-month survival (AOR, 1.33; 95% CI, 1.09-1.61).
【Conclusion】
The on-scene time of EMS for OHCA should be within 7 minutes for the shockable group and within 11 minutes for the non-shockable group. The decision to transport a patient with continued CPR should be made early in the resuscitation process.
