OBJECTIVES: Bromelain-based enzymatic debridement (NexoBrid®) has proven to be an effective, rapid and selective debriding agent in the treatment of adult patients with deep partial and full thickness burns. However, its use in pediatric population is still beyond the manufacturer’s guidelines. We aim to present our experience in the use of enzymatic debridement in pediatric population in a tertiary center.
METHODS: We made a retrospective observational review including all pediatric patients treated with NexoBrid® between June 2019 and December 2021, aged 6 months to 16 years. We included demographic data, %TBSA, burn etiology, complications, number of surgeries, type of analgosedation used for debridement and length of stay.
RESULTS: Enzymatic debridement was carried out in a total of 12 children. Three of them required more than one application due to total burn extension or insufficient total application time. 58.3% required further surgery after debridement because of the depth of the burns. They all were treated at bedside at the Pediatric Intensive Care Unit under regional (n=4) or general anesthesia (n=8). The main adverse event was bleeding after enzymatic debridement, requiring transfusion in three patients. No other adverse effects were registered.
CONCLUSIONS: In our experience, enzymatic debridement with NexoBrid® is a safe and effective procedure in pediatric burned population under the same indications as in adult population and can be considered as a standard of care in the management of acute burns in children as long as it follows adequate and multidisciplinary protocols for its use off-label, adjusting local and global management and analgosedation protocols to burn extension, anatomic location and patient age

We will be pleased to welcome you in Turin (Italy) for the 19th European Burns Association´s Congress in 2022. Italy is honored to hold the meeting of the European Burns Association for the second time, after Verona in 1995. Turin is easily reachable from everywhere in Europe, either travelling by car, train or choosing flight connections. During the meeting, held in the renewed industrial center of the main Italian historical automotive factory (i.e. Lingotto-FIAT), we will meet international burn specialists, focusing on standards of care, progresses in burn treatment, strategies to face disaster situations, with a special attention to education of younger colleagues.
