Atopic dermatitis (AD) is a common chronic or relapsing inflammatory skin condition, often causing a disruption of the skin barrier. AD can be triggered by allergens and is associated with dysregulation of various metabolic pathways. We aimed to characterise the local metabolic profile in lesional and non-lesional skin of AD patients using dermal Open Flow Microperfusion (dOFM). Diluted but otherwise unchanged interstitial fluid (ISF) was collected with dOFM and extracted with cold methanol. Metabolic profiling was done using HILIC-HRMS (Vanquish UHPLC, QExactive mass spectrometer Thermo Fischer Scientific) in positive and negative electro-spray ionisation mode via gradient elution (run time 37 minutes).
Targeted data analysis identified 264 metabolites with sufficient analytical quality for statistical analysis, covering important biological classes, such as lipids, fatty acids or amino acids. Using analysis of variance (ANOVA), a statistically significant increase of phenylalanine, tryptophan and kynurenine was observed in lesional skin compared to non-lesional skin of AD patients. The local metabolic profile displays altered pathways in lesional skin when compared to non-lesional skin of AD patients. Although the mechanism is unclear, our findings suggest that the skin lesion is linked to an affected tryptophan kynurenine pathway. These findings can support the discovery of new biomarkers for clinical and therapeutic approaches.
