Objective
Determine sensitivity of predictive biomarkers of success in pediatric cochlear population
Methods
In 2019, bilateral simultaneous cochlear implantation was introduced from Public Universal Hearing Screening in Catalonia, Spain. Since that date, functional hearing monitoring is carried out with questionnaires and speech perception tests in those children who were diagnosed with profound sensory-neural hearing loss from the hearing screening program. This present study analyses the relationship between aspects explored in the pre-implantation assessment and the performance in auditory functional tests of a group of 80 children who underwent surgery between January 2019 and December 2021 with an age from 11 months to four years old at the time of surgery.
Results
All patients who receive a cochlear implant at the Sant Joan de Déu Hospital undergo a pediatric, neurological, psychological, logopedic, social, and audiological assessment coordinated by the ENT specialist. The cochlear implant committee discusses each case, establishing prognostic factors of evolution for each child. The acquisition of auditory skills was monitored at 3, 6, 12, 18, and 24 months of age. This study analyses the factors that conditioned the development recorded in the examinations for the implant in order to establish the sensitivity of the implant assessment and its relationship with the rate of acquisition of auditory and language skills.
Conclusions
The interdisciplinary evaluation of the pediatric patient with profound hearing loss who receives a cochlear implant is sensitive enough to determine success criteria for each child. Defining biomarkers on pediatric cochlear implantation allows adjust expectations of treatment results. However, considering that the implantation is carried out at a very early age, a significant percentage of our sample presented developmental disorders during the first years of cochlear implant use that could not be expected previously.
