**Main Body of the Abstract**
A 47 year-old male was admitted to our inpatient clinic for scheduled electrical cardioversion of recurrent atrial fibrillation. He had no cardiovascular risk factors; the arrhythmia was an incidental finding. His only complaint was a mild decrease in exercise tolerance.
Because of ECG finding of low voltages and mild left ventricle hypertrophy in a previous echocardiogram in a young patient without history of untreated hypertension, a new echocardiogram was performed. This revealed moderate thickening of left ventricle with granular sparkling aspect of the myocardium consistent with the suspicion of cardiac amyloidosis.
The patient underwent a cardiac-MRI that supported the suspected diagnosis, with post-contrast inversion recovery sequences showing diffuse late-gadolinium enanchement in all cardiac chambers. The exam also showed a large mass at sternal manubrium, so that a whole body CT scan was performed revealing diffuse osteolytic lesions in the skull, pelvis, shoulder blade and ribs in addition to the known sternal mass.
Laboratory exams showed severe proteinuria, significant hypergammaglobulinemia, and monoclonal free light chain in blood and urine samples. The patient subsequently underwent a bone marrow biopsy showing plasma cells infiltrate and green birefringence with polarized light microscopy.
A diagnosis of light chain amyloidosis with cardiac involvement secondary to micromolecular Multiple Myeloma was made.
The case highlights the importance of performing an accurate anamnesis and physical examination as well as careful examination of first level exams in patients presenting with a common condition that could hide a more complex clinical picture.



There is more than meets the eye: an unexpected case of cardiac Amyloidosis. PA61 (#249)

We invite you to attend the XIX International Symposium on Amyloidosis to be held in Rochester, MN.

This symposium is organized as a hybrid meeting (with the focus on in-person) held from Sunday, May 26 through Thursday, May 30, 2024.

Together with the ISA Board and members of Mayo Clinic, a very attractive scientific hybrid program is being planned. The program will cover a variety of types of systemic amyloidosis: ATTR (hereditary and wild-type), AL (light-chain), AA and less frequent forms. There will be key lectures, specific symposia including those company sponsored, oral presentations and poster sessions of accepted abstracts.

COURSE DESCRIPTION
This program will provide a comprehensive review of state-of-the-art education in systemic amyloidosis: basic science, pathophysiology, diagnosis, current and emerging treatments. Scientific presentations will include competitive oral and poster abstracts. The rapid evolution of scientific discovery and expanding treatment options has created tremendous interest in the field and this meeting will fill and important gap in educating scientists and clinicians.
