Introduction and purpose:
The prevalence and significance of right heart dysfunction in patients with cardiac amyloidosis (CA) remains uncertain. The current study sought to establish the extent of right heart involvement using echocardiography and cardiac magnetic resonance imaging (CMR).
Methods:
Patients with confirmed CA by biopsy or cardiac imaging, who had concurrent echocardiography and CMR at Mayo Clinic from 2020 to 2021 were included. Clinical echocardiographic and CMR parameters were collected. Additional CMR parameters analyzed using CVI42 software package.
Results:
A total of 30 patients were included: 22 males (73%), median age was 76 years. 13 (43%) AL and 17 (57%) (16 wild-type and one hereditary ATTR). Five patients have died. By echocardiography, mild or greater RV enlargement was present in 23%, RA enlargement in 54%, moderate or greater TR in 13% and pulmonary hypertension in 23%. By CMR, the median indexed RVEDV was 72 ml/m2, RVEF 49% and RV wall strain -19%. Myocardial delayed enhancement (MDE) was common in LV (77%), RV (48%) and RA (44%).
RVEF at CMR was lower in ATTR than AL (46% v. 54%, p=0.06), as well as RA-EF (34% v. 46%, p=0.017). RVSP was significantly higher among those patients who died (mean 50 mmHg v. 32 mmHg, p=0.005).
Conclusion:
Right heart dysfunction is prevalent in CA. More studies are needed to determine the independent prognostic significance of right heart imaging parameters.

Right heart dysfunction in patients with cardiac amyloidosis PB116 (#362)

We invite you to attend the XIX International Symposium on Amyloidosis to be held in Rochester, MN.

This symposium is organized as a hybrid meeting (with the focus on in-person) held from Sunday, May 26 through Thursday, May 30, 2024.

Together with the ISA Board and members of Mayo Clinic, a very attractive scientific hybrid program is being planned. The program will cover a variety of types of systemic amyloidosis: ATTR (hereditary and wild-type), AL (light-chain), AA and less frequent forms. There will be key lectures, specific symposia including those company sponsored, oral presentations and poster sessions of accepted abstracts.

COURSE DESCRIPTION
This program will provide a comprehensive review of state-of-the-art education in systemic amyloidosis: basic science, pathophysiology, diagnosis, current and emerging treatments. Scientific presentations will include competitive oral and poster abstracts. The rapid evolution of scientific discovery and expanding treatment options has created tremendous interest in the field and this meeting will fill and important gap in educating scientists and clinicians.
