Real-world evidence (RWE) is becoming increasingly relevant for regulatory decisions but can be subject to sources of bias (e.g., baseline confounding) that are less of a concern in randomized clinical trials (RCTs). The results from RCTs are often used as benchmarks for RWE analyses that, once benchmarked, can be extended to populations or interventions not represented in the RCTs. Unfortunately, results from RCTs are often provided under an intention-to-treat approach, which can be severely affected by adherence to the intervention. Here we emulate an existing RCT with low adherence (the PLCO trial) to benchmark RWE using US Medicare data and then proceed to estimate what the effect would have been under complete adherence, which is the estimate of primary interest to regulators, clinicians, and patients.
